Self-exclusion: Voluntarily exclude yourself from gambling activities

The self-exclusion program allows you to voluntarily exclude yourself from gambling in all card rooms statewide.

What the self-exclusion program is

The self-exclusion program, implemented in May of 2022, allows you to submit a single form to voluntarily exclude yourself from all house-banked cardrooms statewide.

How to enroll in the self-exclusion program

The self-exclusion form, once completed and submitted, prevents you from gambling in or being a patron of privately owned house-banked card rooms. Once enrolled, you cannot request early removal.

Are Tribal casinos included in the self-exclusion program?

Tribal casinos are not currently participating in the Washington State self-exclusion program. Please contact each Tribal casino about it's self-exclusion program and monitor this page for updates on participating Tribal casinos.

 

Enroll by mail

  1. Fill out a self-exclusion enrollment form 
  2. Have your form notarized, or ask a certified gambling counselor to sign it
  3. Attach a passport style photo and a copy of your photo ID to your application
  4. Mail your complete application to:

Enroll in-person

Book an appointment online to enroll in the self-exclusion program in-person at our headquarters in Lacey, WA.

Make sure you bring proof of identity to your appointment, such as:

  • Valid driver license
  • Government-issued ID with name, photograph, and date of birth
  • Valid passport.

Enroll at a house-banked card room

If you enroll in the self-exclusion program at a cardroom, your enrollment date starts the day you give your enrollment form to the house-banked card room.

  1. Visit any licensed house-banked card room in Washington State
  2. Request the self-exclusion enrollment Form from staff, complete it, and return it to a house-banked card room employee
  3. The house-banked card room will mail your form in.

How to remove yourself from the self-exclusion program

You will receive a notice 45 days before the expiration of your voluntary self-exclusion period. Once your voluntary self-exclusion period has expired you are required to send in a notarized self-exclusion program removal form to be removed from the program. You will remain in the program until we receive and process your Removal Form.

  2. Have your form notarized.
  3. Mail your complete application to:
