How to apply
Commercial businesses
- Complete the Add-an-Activity - Commercial Business Application (PDF, 250 KB) and any addendums for the activity you would like to add
- Mail your application and fees.
Charitable and nonprofit organizations
- Complete the Add-an-Activity - Charitable / Nonprofit Application (PDF, 250 KB) and any addendums for the activity you would like to add
- Mail your application and fees.
Addendums
Complete, along with the above application, the addendum for the activity you would like to add.
